Instructions 1. Application must be submitted by the grant cycle deadline.2. Any grant not fully completed will not be considered. 3. Submit using this on-line application or print this form and mail to: MPPEF, PO Box 5, Midland Park, NJ 07432 4. Once reviewed, applicants will be notified by MPPEF of acceptance approximately three weeks after the deadline 5. The Grant Committee Chairperson will coordinate fulfillment of the grant with the applicant. 6. Classroom grants must be fulfilled within one year of grant award. 7. Special extension of time may be considered. 8. Applicant must submit a grant evaluation to the Grants Committee no later than 30 days after the completion and /or implementation of the grant.
